Keep this in mind

Ask for the ongoing total, in writing. Put the visit fee, medicine, tests and follow-up on separate lines before comparing plans.

## What might the advertised fee leave out?

In a sample checked on September 26, 2026, most online weight services used memberships and charged separately for medicine.

That sample describes advertised offers, not a price that every service charges.

Is this a visit fee or an ongoing membership?

Is the medicine included?

Are tests, shipping and later reviews included?

What bill remains if the person treating me decides against a prescription?

How does cancellation work, and when does billing stop?

Keep this in mind

The price for your first supply cannot tell you what you will pay in later months.

## A dated guide to medicine-only prices

For semaglutide, the maker listed monthly self-pay prices of $149 to $499 on September 26, 2026. Price depended on which product and strength you chose.

Self-pay means paying outside insurance; a quoted month can mean a different box or bottle depending on the product.

The $199-a-month starting offer for Wegovy and Ozempic injections covered only new patients’ first two monthly supplies at the two lowest strengths. December 31, 2026 was the offer’s end date. After those two supplies, the listed monthly prices were $349 for standard Wegovy injections and $399 for Wegovy HD. Ozempic injections were $349 or $499, depending on strength.

The maker reserved the right to change or cancel its program, so treat those figures as dated context rather than a quote for your care.

Ask the service or insurer for a current total for the exact product being discussed.

Keep this in mind

Before comparing cash and insurance, ask which price is being quoted and what conditions come with it.

## Ask the insurance question precisely

Ask for the formulary, the list of medicines your insurance plan will pay for. For prior authorization, the person prescribing gives the insurer medical details and requests payment approval before the pharmacy fills the prescription.

Medicare coverage depends on the reason for treatment, the plan’s list and its approval rules; being eligible for coverage does not guarantee payment.

Medicare coverage guidance

Will the plan pay for this medicine for my proposed treatment?

Who sends the insurer the information it needs?

What would I owe if the approval is refused?

## Could the temporary Medicare program include you?

Medicare has a temporary option called the GLP-1 Bridge for some people with its drug coverage. For people who qualify, the copay, meaning the amount they pay for a monthly supply, is $50. The program started July 1, 2026 and is scheduled to finish on December 31, 2027.

Medicare Bridge details

GLP-1 is a gut hormone; semaglutide copies its action.

As checked on September 26, 2026, Wegovy tablets and injections were included for weight treatment. Ozempic for weight loss was not included.

Who and what the program covers

Being old enough for Medicare is not enough to qualify. A qualifying Medicare drug plan and an age of 18 or older are both required. The rules consider your health and your BMI at the start of treatment. The letters BMI stand for body mass index, a figure calculated from height and weight. One of these three groups must fit:

A BMI reaching 35, or above.

A BMI reaching 30 or above, together with one of these three health problems. One qualifying form of heart failure has the name “preserved ejection fraction”. With this condition, the heart ejects the usual proportion of the blood held inside it. Another is blood pressure that stays too high despite two blood-pressure medicines. Here, too high means either a bottom number over 90 or a top number over 140. The remaining condition is long-term kidney disease that has reached at least stage 3a. Ask whoever treats you to confirm the stage.

BMI of 27 or higher, plus one of four health problems. The four are prediabetes, an earlier stroke or heart attack, and peripheral artery disease that causes symptoms from narrowed arteries. With prediabetes, blood sugar has risen beyond normal, yet is below the level used to diagnose diabetes.

There are exclusions too. Type 2 diabetes, moderate or severe obstructive sleep apnea, or a particular fatty liver disease called MASH can send you to ordinary Medicare drug coverage instead. In obstructive sleep apnea, a blockage interrupts breathing again and again while you sleep. The liver exclusion covers MASH when scarring is moderate to advanced and cirrhosis is absent. Cirrhosis means the most advanced scarring, so the exclusion does not cover every fatty liver diagnosis. Prescriptions to lower heart-attack or stroke risk also go through ordinary drug coverage.

Prior payment by Medicare Part D for a GLP-1 medicine in 2026 blocks Bridge use in 2026. Medicare’s usual benefit for prescription medicines is called Part D. The sources checked had not yet set that rule for 2027. A prescription and approval in advance are required, along with ongoing help with eating and physical activity.

Ask the person prescribing to check your exact diagnoses and plan type against the linked program rules. These groups are a starting point for that check, not a promise of approval.

The program gives no extra help based on low income. The $50 payments do not count toward your usual Medicare drug-plan deductible, the amount paid before coverage starts, or its yearly limit on what you pay.

Limits on what the copay counts toward

Check which way of paying applies to your treatment before treating $50 as your own monthly price.
